Why a few centimeters change the reading
An automatic monitor works by inflating a bladder inside the cuff until it briefly stops blood flow in the main artery of your upper arm, then sensing the pulses as the cuff slowly deflates. For that to work as designed, the bladder has to wrap far enough around the arm to squeeze the artery evenly. The 2017 American College of Cardiology and American Heart Association guideline specifies that the bladder should encircle about 80 percent of the arm.
When the cuff is too small for the arm, the bladder covers too little of the circumference. It has to inflate harder to compress the artery, and the monitor reads that extra pressure as higher blood pressure. When the cuff is too large, the opposite happens and readings run low.
Suppose your true systolic pressure is 128 mm Hg, which falls in the range the 2025 AHA and ACC guideline calls elevated. A regular cuff on an arm that needs an extra large cuff could show a number near 147 mm Hg on average, which is in the range the guideline calls stage 2 hypertension. The monitor has not malfunctioned. It has been asked to measure through the wrong cuff. A diagnosis always depends on repeated readings interpreted by a health care professional, but you can see how an ill fitting cuff could send the wrong signal.
Errors in the other direction matter too. Kenny Cole, an internist at Ochsner Health, told the American Medical Association in March 2026 that a cuff too big for a small arm can produce falsely low readings. A falsely low number can be more troubling than a falsely high one, because it can leave high blood pressure looking normal.
What you need
- A flexible tape measure, the cloth or vinyl kind used for sewing. A metal carpenter's tape does not bend around an arm well. If you do not own a flexible tape, wrap a piece of string or a phone charging cable around your arm, pinch it where it meets, and lay it flat against a ruler.
- A pen or washable marker to mark the midpoint of your arm.
- A helper, if possible. Measuring your own upper arm with one hand is awkward, and it is easy to let the tape sag or tilt. A family member can do it in seconds.
- A short sleeve or sleeveless top, so you measure skin and not fabric.
Measure the arm you plan to use for readings. The 2017 guideline advises measuring blood pressure in both arms at a first evaluation and then using the arm with the higher readings. If you have not settled that yet, measure both arms and write down both numbers. Arms are not always the same size, and if the two land in different cuff ranges you will want to know before you buy.
Finding the right spot
The measurement is taken around the middle of the upper arm, halfway between the shoulder and the elbow. The American Medical Association describes the landmarks precisely. At the top, feel for the acromion, the bony point at the outer tip of your shoulder. At the bottom, feel for the olecranon, the hard point of your elbow.
Bending the elbow to a right angle makes the elbow point easier to find. Once you have both landmarks, measure the distance between them along the outside of the arm and divide it in half. That halfway point is where the tape goes around. Mark it with your pen.
Then let the arm hang relaxed at your side. One manufacturer's instructions say specifically to measure with the arm hanging, and the detail matters. Flexing the biceps or lifting the arm changes the shape of the muscle and can change the number.
Step by step
- Stand or sit upright with the arm bare and hanging loosely at your side.
- Find the shoulder point and the elbow point, measure the length between them, and mark the halfway point on the arm.
- Wrap the tape around the arm at the mark. Keep it level all the way around, not angled up toward the shoulder on one side.
- Bring the tape until it lies flat against the skin without pressing in. One manufacturer's fitting guidance says the tape should not be pulled tight. If it dents the skin, loosen it.
- Read the number in centimeters. Most cuffs and sizing charts, including the standard table used by the AHA and the AMA, use centimeters. If your tape shows only inches, multiply by 2.54.
- Measure a second time. If the two readings differ, take a third and use the number that repeats.
- Write the result down where you will find it again, such as a note on your phone. You will want it when you compare cuffs, and again later if you replace the cuff or share the monitor with someone else.
The whole process, including finding the landmarks, usually takes less time than reading this section.
Reading the size ranges
The AMA publishes a cuff size table adapted from the AHA's 2019 scientific statement on blood pressure measurement, and the 2017 guideline lists the same four adult ranges.
- Small adult: 22 to 26 cm, about 8.7 to 10.2 inches.
- Adult: 27 to 34 cm, about 10.6 to 13.4 inches.
- Large adult: 35 to 44 cm, about 13.8 to 17.3 inches.
- Extra large adult: 45 to 52 cm, about 17.7 to 20.5 inches.
These labels help you understand where your arm falls, but do not shop by them. Manufacturers do not agree on what "standard," "large," or "wide range" means, and the ranges printed on their cuffs are what count. One company sells a small cuff for 16 to 24 cm, a medium cuff for 23 to 37 cm, a large cuff for 31 to 45 cm, and a wide range cuff for 22 to 42 cm. Another company labels its cuffs in inches: small 7 to 9, standard 9 to 13, large 13 to 17, plus a single expandable cuff spanning 9 to 17 inches, roughly 23 to 43 cm. That expandable cuff, or one like it, is what comes in the box with many monitors sold in the United States.
So a 36 cm arm is a "large adult" arm in the standard table but sits comfortably inside a "medium" 23 to 37 cm cuff and inside the common 9 to 17 inch cuff. A 44 cm arm is also "large adult" in the table, yet it falls outside that medium cuff entirely and lands at the very top of the 9 to 17 inch cuff. The word on the box tells you little. Your centimeter number against the printed range tells you what you need to know.
Michael Rakotz, a family physician at the AMA, made the practical point in the same March 2026 article: home monitors usually come with one standard cuff, so the question to answer before buying is whether that particular cuff fits the middle of your upper arm. If it does not, the monitor will not give accurate readings, however good the device is.
When your number lands near the edge
Cuff ranges overlap, and many people land in the overlap. A 42 cm arm (about 16.5 inches) sits at the very top of a 22 to 42 cm cuff, inside a 31 to 45 cm cuff, and at the very bottom of a 42 to 60 cm cuff. All three technically fit.
Manufacturer guidance is that when you fall between two sizes you can choose either, as long as you then use the same cuff for every reading. Consistency matters because a home blood pressure record is only useful when readings can be compared with one another over weeks and months. Switching between two cuffs of different sizes adds noise to your record that has nothing to do with your health.
If you have a choice, a cuff whose range puts your arm closer to the middle rather than the last centimeter gives you some margin. Arm circumference changes with weight gain or loss, with fluid retention, and with muscle changes as people age. An arm that measures 43 cm today may measure 41 or 45 cm next year. Remeasure after any noticeable change in weight, and remeasure anyway when you replace a cuff.
Confirming the fit when the cuff arrives
Your measurement gets you the right cuff. The cuff itself then confirms it. Many cuffs carry two printed marks: a range zone, often shown as a band or a pair of arrows, and an index line or arrow near the end of the cuff. Wrap the cuff around your bare upper arm with the bottom edge just above the bend of the elbow and the artery marker (usually an arrow or line near the tube) over the inside of the arm. When the index line falls inside the range zone, the cuff is the right size for you. If the line stops short of the zone, the cuff is too small. If it passes beyond the zone, the cuff is too large.
Fit should feel snug and even. The AHA's 2019 statement describes the correct fit as snug with similar tightness at the top and bottom edges. One manufacturer's guidance for its ring style cuffs is that you should be able to slip one finger between the cuff and your arm. A cuff that barely closes, or whose fastener catches only at the tip, is too small no matter what the printed range says. A cuff that bunches, slides down, or leaves a loose gap at the top is too large or wrapped too loosely.
Then bring the monitor to your next appointment. The AHA recommends having your health care professional watch you take a reading, check that you are using it correctly, and compare its results with the office equipment. A cuff that fits well is the first thing they will look for.
If your arm is larger than most cuffs
Cuffs for very large arms are harder to find than the standard table suggests. The extra large adult range runs from 45 to 52 cm, yet the widely sold expandable cuff described earlier stops at 17 inches, about 43 cm, and that company sells no larger upper arm cuff. The AMA notes plainly that not all devices offer extra large cuff sizes.
At least one manufacturer sells a home monitor with a cuff for arms 42 to 60 cm (about 16.5 to 23.6 inches), which covers the full extra large range and beyond. Options like that exist, but there are fewer of them and they tend to cost more than a basic monitor, so knowing your number before you shop saves an exchange or a return.
Two details matter when you buy for a larger arm. First, check that the exact model with the larger cuff appears on the US validated device listing at validatebp.org rather than assuming a standard model's listing carries over. The listing criteria require that the cuff sizes were tested with the device and are available for the people it is meant for. A companion article in this section explains how to check a listing. Second, if a company sells a larger cuff as a separate accessory, confirm that it is made for your specific monitor model. Cuff connectors differ between models, and a cuff sold for one monitor may not attach to another.
Some people with very large arms cannot find an upper arm cuff that fits at all. The AHA's 2019 statement expresses strong reservations about routine use of wrist monitors but allows for them when upper arm measurement is not feasible, and heart.org notes that wrist devices give less reliable readings. If a wrist device is the only option, it measures correctly only when the sensor sits directly over the artery on the inside of the wrist and the wrist is held at heart level. Readings run high when the wrist is below the heart and low when it is above. This is a decision worth making with your care team, who can compare a wrist device against the office equipment before you rely on it.
If your arm is smaller than the standard cuff
The other end of the range gets less attention, but it affects many older adults and small framed people. Standard cuffs typically start at 22 or 23 cm, around 9 inches. An arm below that is common in someone who has lost weight or muscle with age. On such an arm, a standard cuff reads low. The Cuff(SZ) trial found an average of 3.6 mm Hg too low for people who needed a small cuff, and individual results varied more than the average suggests.
Small adult cuffs cover roughly 22 to 26 cm in the standard table. Some manufacturers sell cuffs that go smaller, for example 16 to 24 cm, and one sells a small ring style cuff for arms 7 to 9 inches (about 18 to 23 cm). These are usually not the cuff in the box, so you will need to look for a monitor that either includes a small cuff or is sold in a version with one.
If you are buying for a teenager or a child, cuff size is only part of the question. The AHA advises choosing a monitor that has been validated for the person who will use it, and a device validated in adults may not have been tested in younger people or during pregnancy.
Before you check out
- Have your arm measurement in centimeters written down, for each person who will use the monitor.
- Find the cuff range on the product listing, the box, or the downloadable manual, and confirm it includes your number, ideally not at the very last centimeter.
- Confirm that the exact model number, with the cuff you are buying, appears on validatebp.org.
- If your arm is outside the included cuff's range, check whether the manufacturer sells a cuff in your size for that model, or look for a model that includes one.
- If two people will share the monitor and their arms fall in different ranges, plan on two cuffs or two monitors. Another article in this section covers sharing a monitor in more detail.
- When the monitor arrives, wrap the cuff and check that the index line falls in the range zone before taking your first reading.
- Take the monitor to your next appointment so your health care professional can watch your technique and compare readings with the office equipment.
Keep the tape measure somewhere you can find it. You will want it again the next time you replace a cuff.
