Sleep and daily routines

Preparing sleep questions when you work changing shifts

Most home blood pressure instructions assume a life you do not have. Take two readings in the morning before your medicines, take two more in the evening before supper, repeat for a week. If you clock out at 7 a.m., sleep until 3 p.m., and eat your first real meal at 4, none of those anchors mean anything.

That mismatch is worth raising at your next appointment, because the answer is not obvious and your care team has to choose it with you. It is much easier to settle if you walk in with two weeks of notes showing your shifts, your sleep, and your readings together on the same page. Everything below is built around that record: what to write down, what to ask, and where the line falls between what you can decide on your own and what needs a health care professional.

What is actually known about changing shifts and blood pressure

The honest summary is narrower than most articles suggest. The National Institute for Occupational Safety and Health, part of CDC, states that shift work "possibly increases risk for cardiovascular diseases such as myocardial infarction, chest pain, and high blood pressure." Myocardial infarction is a heart attack. The word "possibly" is doing real work in that sentence, and NIOSH keeps it there on purpose.

NIOSH also notes that the research links are stronger for the factors that promote heart disease than for the diseases themselves. Put simply, studies more consistently show shift work shifting the things that lead to cardiovascular disease than show it causing the disease outright.

Four biological pathways get named. Decreased glucose tolerance means the body handles sugar from a meal less efficiently, especially at night. Insulin resistance means cells respond less to the hormone that moves sugar out of the blood. Elevated cortisol means a stress hormone that normally rises and falls on a daily rhythm gets pushed out of its usual pattern. Increased sympathetic nervous system activity means the alerting branch of the nervous system stays switched on more of the time, which raises heart rate and tightens blood vessels.

None of that proves your schedule has changed your numbers. It explains why a health care professional may want to look more closely, and why NIOSH recommends periodic health screening for shift workers rather than waiting for symptoms.

CDC states separately that getting enough sleep is associated with lower risk of several chronic conditions, high blood pressure among them. CDC's recommended amounts are given per day, not per night, which matters when your sleep lands in the afternoon: adults 18 to 60 need 7 or more hours, adults 61 to 64 need 7 to 9 hours, and adults 65 and older need 7 to 8 hours.

Build two weeks of notes that show the shift

A record that only lists blood pressure numbers hides the thing your health care professional needs to see. Add the schedule and the sleep, and the same numbers start to mean something.

For two weeks, write down:

  1. When each shift started and ended, and whether the day was a work day or a day off.
  2. When your main sleep period started and ended, including sleep that runs across the afternoon, and any naps with their rough length.
  3. Every blood pressure reading with the clock time beside it and how long you had been awake.
  4. Caffeine: roughly how much and at what hours, including the cup you drink at 2 a.m.
  5. Alcohol: how much and at what hour, especially if you use it to fall asleep after a shift.
  6. Anything that woke you and anything anyone else noticed, such as snoring or pauses in your breathing.

Two weeks is the useful window for a reason. If your schedule rotates, one week may capture only the day stretch and tell your health care professional nothing about the nights. Estimates are fine. Nobody needs you checking the clock at 3 a.m., and watching the clock tends to make sleep worse. Where the usual home monitoring advice breaks down

The standard schedule comes from the American Heart Association's scientific statement on blood pressure measurement: two readings at least a minute apart in the morning before blood pressure medicines, and two more in the evening before bed, ideally for 7 days and no fewer than 3, then average them. The logic is that one set lands near waking and one lands late in the day, which brackets the normal rise and fall.

For a night worker, "morning" is a clock word, not a body word. The sensible translation is that one set belongs shortly after your main sleep period ends and one belongs near the end of your waking hours, whatever the clock says. Do not decide that on your own. Ask which two times your care team wants, then keep those times constant across both halves of your rotation and write the shift beside each reading.

Technique matters more on a rotating schedule than it does for anyone else, because you are often measuring right after a commute, a coffee, or a flight of stairs. Sit with your back supported and feet flat, empty your bladder first, rest quietly for at least 5 minutes, keep the cuff on a bare upper arm with the middle of the cuff at heart level, rest the arm on a flat surface, and do not talk or scroll while the cuff inflates. Skip caffeine, smoking, and exercise for at least 30 minutes beforehand.

Two numbers help you read your own log without overreacting. Home readings run lower than office readings for the same person, so an office reading of 140/90 mm Hg corresponds to a home average near 135/85 mm Hg. And a single reading is not enough to decide anything. The 2025 American Heart Association and American College of Cardiology guideline defines its categories using an average of two or more careful readings taken on two or more occasions, interpreted by a health care professional.

Questions about medicine timing when the schedule rotates

This is the question shift workers most often want answered and most often answer themselves. Do not.

There is useful evidence to bring into the conversation. The 2025 guideline reviewed two large randomized trials comparing morning with evening dosing of blood pressure medicines and found no difference in cardiovascular outcomes between them, and it notes that one large trial found better adherence with morning dosing. Translated: for most people the clock time appears to matter less than whether the dose actually gets taken. That reframes your question from "which time is better" to "which time will I hit on every rotation."

Questions worth writing down before the visit:

  1. If my schedule rotates, should I take my blood pressure medicines at the same clock time every day, or tie them to when I wake up?
  2. What do I do on the changeover day, when yesterday's dose was at 8 a.m. and today I am asleep at 8 a.m.?
  3. Do any of my medicines make me drowsy or lightheaded, and does that change anything about driving home after a shift?
  4. Do any of them need food, and how does that work when my main meal is at 3 a.m.?
  5. Because I sleep during the day, would a 24 hour ambulatory monitor give a more accurate picture than my home readings?

That last one is a fair question rather than a demand. Ambulatory monitoring, which records readings automatically over a full day and night, can capture what happens during sleep. Whether it is worth doing in your case is a clinical judgment.

Bring the rest of the shelf into the conversation too. The 2025 guideline lists substances that can raise blood pressure or interfere with treatment, including alcohol, caffeine with a suggested limit under 300 milligrams a day, decongestants such as phenylephrine and pseudoephedrine, nonsteroidal antiinflammatory pain relievers, and several prescription drug classes. About 20 percent of adults with high blood pressure regularly use nonprescription medicines that fall into that group. Tell your health care professional about every medicine, vitamin, and supplement you take, and be specific about the caffeine, since "a few cups" and 500 milligrams are different conversations.

Sleepiness and tiredness are different, and clinicians hear them differently

People use the words interchangeably. health care professionals do not. Sleepiness means you actually doze off, or would if you sat still. Tiredness means low energy without dozing. They point toward different causes and different next steps, so say which one you mean.

A health care professional may use a short questionnaire in which you rate how likely you are to doze off in eight ordinary situations, then adds up and interprets the answers. The best known of these is the Epworth Sleepiness Scale, which appears in NIOSH's own training for nurses on shift work. It is a conversation starter for a health care professional, not a self test, and the number only means something alongside your history.

What to report, in your own words:

  1. Which situations you doze in, and whether that includes driving or sitting in stopped traffic.
  2. How many hours you sleep on work days compared with days off, in both halves of your rotation.
  3. How many times you wake during your main sleep period, and whether you can get back to sleep.
  4. Whether you wake with a headache, a dry mouth, or a sore throat.
  5. What you take in the hours before sleep, including alcohol.

Driving deserves a plain sentence. The National Heart, Lung, and Blood Institute states that sleepiness while driving, unrelated to alcohol, is responsible for serious car crash injuries and death. If you have drifted across a lane line on the way home, or caught yourself nodding at a light, say that out loud at the appointment. It changes the conversation, and it is not something to push through.

Sleep problems that deserve their own visit

Sleeping badly because your body clock is pointed the wrong way is not the same as a sleep disorder, though the two can sit on top of each other. NHLBI's definition of sleep deficiency covers both: not getting enough sleep, sleeping at the wrong time of day, or not getting the kinds of sleep the body needs.

Two conditions come up often enough to name.

Snoring and sleep apnea. The National Institute on Aging is clear that snoring can be a sign of sleep apnea and that plenty of snoring is not. What raises a health care professional's interest is snoring that stops and restarts, someone witnessing pauses in your breathing, gasping or choking, and daytime sleepiness that does not fit your sleep hours. Sleep apnea is common in people whose blood pressure is hard to control, and treating it is worth discussing. Here is the nuance almost every article gets wrong: in 2022 the US Preventive Services Task Force concluded that evidence is insufficient to assess screening for obstructive sleep apnea in the general adult population. That statement applies to adults without symptoms who have not raised a concern. If you snore loudly, wake gasping, or doze during the day, you are outside that population by definition, and the finding is not a reason to stay quiet. Insomnia. The National Institute on Aging calls insomnia the most common sleep problem in adults 60 and older, defined by trouble falling or staying asleep at least three nights a week. The most useful fact here surprises most people: the American College of Physicians recommends cognitive behavioral therapy for insomnia, sometimes written as CBT I, as the initial treatment for chronic insomnia in all adults, and grades that a strong recommendation. Its recommendation about adding medication is weaker, and it asks that the decision be made jointly after discussing benefits, harms, and costs of short term use. The National Institute on Aging puts it plainly as well: sleep medicines may help in the short term, but they carry risks. Never start, stop, or retime a sleep medicine on your own, prescription or not.

Where your judgment ends

Some of this is yours to decide. You choose when to make your sleep block, how dark and quiet you can make the room, when you cut off caffeine before sleep, whether you keep the log, and whether you ask for the appointment near the end of a rotation so the record covers both halves.

Some of it is not. A diagnosis of high blood pressure rests on repeated readings interpreted by a health care professional. Changing the timing or dose of any medicine, ordering a sleep study, treating insomnia with a drug, and judging whether your sleepiness is safe for the work you do are all clinical calls, and some involve your employer's occupational health rules as well.

One threshold is worth memorizing. If a reading is 180/120 mm Hg or higher, wait at least a minute and take it again. If it stays that high and you have no symptoms, contact your health care professional as soon as possible. If it stays that high and you have symptoms such as chest pain, shortness of breath, back pain, numbness or weakness, a change in vision, or trouble speaking, do not wait to see whether it comes down. Call 911. NIOSH gives shift workers a similar instruction in its own words: get medical attention for shortness of breath, an irregular heartbeat, chest pain, or high blood pressure.

Write your questions on one page and hand it over at the start of the visit rather than the end. Fifteen minutes goes fast, and the question about when to take your readings is the one that makes every future appointment more useful.
